Admission Procedures Phase III
After completion of Phase II and attendance at the Mandatory Preliminary Meeting, final acceptance into the program (Phase III) is contingent upon:
- Two-step PPD test
- Ten Panel Drug Screen (instructions will be provided)
- Pennsylvania Child Abuse History Clearance *
- Pennsylvania Criminal Background Check - PA State Police *
- FBI Fingerprint Clearance *
- CPR Healthcare Provider Certification
- Receipt of all required documents
- $150.00 non-refundable acceptance fee (To pay, please go to Student Accounts, Newtown Campus. You must identify yourself as a Practical Nursing Student)
* Admission is denied for prior history of any felony conviction.
IMPORTANT: DO NOT begin acquiring the Phase III documents until you attend the Mandatory Preliminary Meeting.
Below is helpful information as you complete these forms:
1. Pennsylvania Child Abuse History Clearance
a. Be sure to complete the online application, not the paper version. Under Purpose of Clearance select VOLUNTEER/OTHER
b. Be sure all the occupants of your household are listed.
c. Make a copy for yourself and bring in or send us the original.
2. Pennsylvania Criminal Background Clearance
a. Most results can be printed immediately
b. If your copy only has a control number but not a record determination i.e. “no record exists” or a printout of prior offenses, you need to go back to the Epatch website. Please follow the steps below to gain access to your record:
i. Select Record Check
ii. Select Check Status
iii. Type in your Control Number, Name (case sensitive) and Date of Request
iv. Click on the Record Check Details screen
v. At the bottom of the page, click on Certificate Form.
vi. Print the certificate (be sure the state watermark seal appears).
vii. Make a copy for your records
3. FBI Fingerprint Clearance
a. Log into above website (chrome preferred).
b. Enter the following service code: 1KG6ZJ and hit “GO”.
c. Select “Schedule or Manage an Appointment” and follow steps to complete your application. *Note: Schedule an appointment, DO NOT choose “walk-in” as an option.
d. Results will be mailed to your home within approximately 2 weeks. Submit your original results, along with the receipt, to the Practical Nursing Program. Make a copy for your records.
4. Two Step PPD Immunization Record:
a. Remember this is a 2-step process and must be completed accurately on the form provided.
b. Interferon-Gamma Release Assay (IGRA) is accepted in place of the Two-Step PPD. Either of the two IGRA’s (Quantiferon Gold or T-Spot) are approved.
c. Allow up to 2 weeks to complete the test.
d. Send the original to the Practical Nursing office. Be sure to keep a copy for your records.
e. The results must be submitted 1 week before orientation.
f. Make a copy for your records
5. Professional Healthcare Provider CPR Certification:
a. When registering for your CPR certification make sure you are registering for the Basic Life Support BLS Professional Health Care Provider Certification.
b. Certification courses are offered at Bucks’. See enclosed flyer.
c. If you already have a Professional Health Care Provider Certification it must be valid for the entire program year.
d. You must present your ORIGINAL card to the Practical Nursing program prior to the program start date
e. Make a copy for your record
